  • Describe the specific outcomes associated with your project. Your outcomes are what you expect to measure. The outcomes should link to the problem
• Detail Plan for Evaluation of Project Effectiveness
  • Introduce the need for planning for project evaluation in the design of an evidence-based practice change project (May use attached Resources along with additional if needed)
• Detail Planned Demographic Data Collection (What Data will you collect)
  • Describe the plan for collecting and analyzing your selected demographic points. [Keep in mind the purpose of the demographic data is to describe your participant group only.]
