- Describe the goals of your project. Your goals will not be measured and should reflect the long range, high level purpose of your project
- Detail Planned Measurable Outcomes
- Describe the specific outcomes associated with your project. Your outcomes are what you expect to measure. The outcomes should link to the problem
- Detail Plan for Evaluation of Project Effectiveness
- Introduce the need for planning for project evaluation in the design of an evidence-based practice change project (May use attached Resources along with additional if needed)
- Detail Planned Demographic Data Collection (What Data will you collect)
- Describe the plan for collecting and analyzing your selected demographic points. [Keep in mind the purpose of the demographic data is to describe your participant group only.]
- Discuss specific data points you will be collecting (ie age, gender etc)
- Discuss your plan for collecting the demographic data (i.e pre-post test questionnaires, surveys)
- Discuss your plan for analyzing the collected demographic data (mean and range will be calculated for age, percentage will be calculated for gender etc)

- The tool/data collection source that will be used (i.e self-efficacy screening scales, charts etc.)
- The approach you will utilize to analyze the data meaning (descriptive statistics to calculate pre-post means, percent change (ie Cronbach’s alpha, predictive validity, central change etc)
- The Benchmarks that will be set as a goal for outcome attainment (i.e 30% increase in screening participation, compliance, adherence etc)
- Repeat the above steps for each outcome (Label as Outcome 1, Outcome 2 etc)

- Thoroughly describe how you will:
- Protect confidentiality (ie random codes)
- How data will be stored and kept confidentially secure (ie excel spreadsheet on password encrypted computer)
- Who will have access to data (i.e Only project investigator, manager and on request faculty advisor)
- Thoroughly describe how you will:
- Conclusion (Summarize main purpose of the paper and key aspects of the project plan)
